About the Service
The Managed Clinical Network (MCN) is a co-ordinated network of people involved in diabetes care across the region. Within this network health and social care professionals and people with diabetes work together to continuously develop and improve diabetes care. This website provides local information about diabetes and diabetes care for both professionals and people with diabetes.
Diabetes is a long-term and progressive condition that can potentially have damaging effects on health. However, evidence shows that many of the complications associated with diabetes can be prevented or delayed through a combination of foot and eye screening, attending appointments with your health care professional, self managing your condition and the appropriate use of medicines and technology i.e. my diabetes my way and blood glucose meters. Therefore annual assessment of these processes of care is important.

Diabetes 9 Processes of Care
Are you aware of the 9 key (age appropriate) processes of care measurements for people living with diabetes should expect to receive on an annual basis?
The 9 key processes of care measurements are:
- Weight & BMI
- Blood Pressure
- HbA1c
- Retinopathy Screening *
- Foot Risk stratification *
- Urinary Albumin test
- Serum Creatinine
- Smoking Status
- Cholesterol level
*this may be every 2 years depending on your results
